Cyndie Baker
Cyndie graduated from college in 1982 with a degree in Marketing. She has previous experience managing the supplement department at a local vitamin and supplement store in Fishers, IN. Her goal when hired at Dr. Aziz Pharmacy in 2020 was to continue to provide exceptional patient care and assist our Primary Care Specialist, Tanya, with whatever task she may need. When not at work, you can find Cyndie spending time with her family and grandkids or finding antiques to add to her collection.

Primary Care Specialist
Tanya Smith, PA-C
Tanya graduated with a Bachelor's of Science in Physician Assistant Studies from Butler University College of Pharmacy and Health Sciences in 2001. She later received her MBA in 2011, became a Certified Medical Examiner in 2017, and is currently CPR certified and NCCPA Board certified. Tanya began her journey with Dr Aziz Clinic in 2016, bringing with her over 10 years of experience in primary care. She is passionate about helping her community stay healthy through customizable medical experiences. When not at the clinic, Tanya enjoys reading, partaking in outdoor activities, learning about art, and spending time with her friends and family.
Registered Nurse, Educator, Researcher
Danielle Perkins Manning
Dr. Perkins Manning is an advanced practice registered nurse, educator, and researcher with over 20 years of experience. She has experience with general medical-surgical nursing, behavioral health nursing, and deep passion for mental health. After earning her Ph.D. in nursing in 2013, she worked on research investigating depression in formerly incarcerated young black men. After completing her Psychiatric Nurse Practitioner education at the University of Southern Indiana in Evansville in July 2022, she became board-certified that August. In her spare time, Danielle enjoys spending time with her family, reading, listening to music, and spending time outdoors.

Leslie Fahey, RN
Leslie graduated from IVY Tech State College in 2001 with her Associate's degree in Nursing. She joined the Dr Aziz team in 2021, bringing with her a diverse background in various specialties and over 20 years of experience as a Registered Nurse. From assisting in surgeries to working with mental health, her passion for nursing stems from the opportunities it provides and her desire to give her patients the best possible care. When not at the pharmacy, Leslie enjoys staying active with biking, working out, playing soccer and golf, and likes to cook and listen to live music.
